Summary: By using our website or purchasing any service, you agree to these terms. The Longevity Clinic is a health coaching service — not medical care. The CALS program requires professional-level output across 15 months. Payments are processed by Stripe. Memberships can be cancelled anytime. CALS program fees are non-refundable once evaluation has begun.

1. Agreement to Terms

These Terms of Service (“Terms”) govern your access to and use of the website at www.floridauhs.org and all services offered by Florida University of Health and Sciences (“FUHS,” “we,” “us,” or “our”), including the Longevity & Preventive Health Clinic and the Certificate in Applied Longevity Systems (CALS) program.

By accessing our website, submitting a contact form, or purchasing any service, you agree to be bound by these Terms. If you do not agree to these Terms, please do not use our website or services.

We reserve the right to update these Terms at any time. Continued use of our services after any update constitutes acceptance of the revised Terms.

2. Important Health Disclaimer

The Longevity Clinic is a health optimization coaching and education service. It is NOT a medical clinic. FUHS does not diagnose, treat, cure, or prevent any disease or medical condition. Nothing on this website or provided through our services constitutes medical advice. All protocols, recommendations, and information are for general wellness and educational purposes only. Always consult a qualified physician or licensed healthcare provider before beginning any new health program, supplement regimen, dietary change, or exercise program. Do not disregard professional medical advice or delay seeking it based on anything you read or receive from FUHS.

3. Longevity Clinic — Membership Terms

Membership plans

We offer three monthly membership tiers: Foundation ($79/month), Optimized ($199/month), and Clinical Bridge ($349/month). Each plan includes the services described on our website at the time of purchase.

Billing

Memberships are billed monthly on a recurring basis through Stripe. By purchasing a membership, you authorize FUHS to charge your payment method on a monthly basis until you cancel. All prices are in US dollars.

Cancellation

You may cancel your membership at any time. Cancellations take effect at the end of the current billing period — you will retain access to your membership benefits through the end of the period for which you have already paid. To cancel, contact us at floridauhs@gmail.com or manage your subscription through the Stripe customer portal.

Refunds

Monthly membership fees are non-refundable once a billing period has begun. If you cancel mid-month, you will not receive a prorated refund for the remainder of the month, but you will not be charged for subsequent months. We reserve the right to issue refunds on a case-by-case basis at our sole discretion.

Service modifications

We reserve the right to modify, suspend, or discontinue any aspect of the Longevity Clinic at any time. We will provide reasonable notice of any material changes to services included in your membership. If we make changes that materially reduce your benefits, you may cancel without penalty.

At-home lab panels

At-home biomarker lab panels are provided through third-party laboratory partners. FUHS facilitates access to these services but is not responsible for the accuracy of lab results, shipping delays, or any issues arising from third-party laboratory services. Lab results are for wellness tracking purposes only and do not constitute a medical diagnosis.

4. CALS Program — Certificate Terms

Program overview

The Certificate in Applied Longevity Systems (CALS) is a 15-month, honor-based, self-directed certificate program. Enrollment is confirmed upon receipt of the $100 administrative enrollment fee and return of the completed enrollment form.

Fee schedule and payment

The total program cost is $1,000, paid in stages as work is submitted:

Textbooks from the 100-Year Series are purchased separately at retail pricing through Amazon or Barnes & Noble and are not included in the program fee.

Refund policy — CALS

The $100 administrative enrollment fee is non-refundable once the enrollment form has been processed and program materials have been provided. Research paper and manuscript evaluation fees are non-refundable once evaluation has commenced. If you withdraw from the program before submitting a paper for evaluation, any unapplied fees may be credited toward a future enrollment at FUHS’s sole discretion.

Evaluation and passing standard

Each research paper is evaluated against a 100-point rubric. A score of 70 or above is required to pass. Papers scoring below 70 receive detailed written feedback and may be resubmitted once upon payment of the $50 resubmission fee. The final manuscript is evaluated against an expanded rubric ($400 evaluation fee) and may be resubmitted once upon payment of the $100 manuscript resubmission fee.

Certificate issuance

The Certificate in Applied Longevity Systems is issued by Florida University of Health and Sciences upon satisfaction of all five requirements: five passing research papers, a passing final manuscript, a published book with valid ISBN, proof of publication, and full payment of all program fees. FUHS reserves the right to withhold certification if any requirement is not met.

Honor system

The CALS program is honor-based. All submitted work must be the original work of the enrolled student. Submission of work that is not your own, including AI-generated content submitted as original research, constitutes a violation of program integrity and may result in immediate dismissal from the program without refund.

Program modifications

FUHS reserves the right to modify program requirements, evaluation criteria, and fee schedules. Enrolled students will be notified of any material changes and will not be subject to retroactive changes to requirements already completed.

5. Payments and Billing

All payments are processed by Stripe, Inc. By making a payment, you agree to Stripe’s Terms of Service. FUHS does not store your payment card information. All financial transactions are subject to Stripe’s security and fraud prevention measures.

You represent that you are authorized to use the payment method you provide and that the payment information you provide is accurate and complete.

6. Intellectual Property

All content on www.floridauhs.org — including text, design, graphics, logos, and program materials — is the property of Florida University of Health and Sciences and is protected by applicable copyright and intellectual property laws.

CALS program participants retain full ownership and copyright of all research papers and manuscripts they produce as part of the program. By submitting work for evaluation, you grant FUHS a limited, non-exclusive license to review and evaluate your work for program purposes only.

You may not reproduce, distribute, or use any FUHS content for commercial purposes without our express written permission.

7. Limitation of Liability

To the fullest extent permitted by applicable law, FUHS, its officers, directors, employees, and agents shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of our website or services, including but not limited to:

Our total liability to you for any claim arising out of or relating to these Terms or our services shall not exceed the total amount you have paid to FUHS in the 12 months preceding the claim.

8. Governing Law

These Terms are governed by and construed in accordance with the laws of the State of Florida, without regard to its conflict of law provisions. Any dispute arising from these Terms or your use of our services shall be resolved in the courts of Putnam County, Florida.

9. Eligibility

Our services are available to adults 18 years of age and older. By using our services, you represent that you are at least 18 years old and have the legal capacity to enter into these Terms.
